The California's Coordinated Care Initiative (CCI) is designed to deliver high-quality, integrated care to low-income seniors and Californians with disabilities.
In this workshop, learn information on Cal MediConnect, managed Medi-Cal plans that incorporate Long-Term Services & Supports, and the Program of All-Inclusive Care for the Elderly (PACE). Also, find out how these programs offer various levels of coordinated care and extra support to help adults who qualify maximize their Medicare and Medi-Cal benefits and get the care they need.
