Review what is an Advance Directive and what is a Physician’s Order for Life Sustaining Treatment (POLST) form is and how they are used to give direction. We will have time for Q&A as well.

Speakers:
Dr. Esther Luo: Esther graduated from medical school in 2006 from The University of California, Davis. She was then an intern, resident, and chief resident in internal medicine at Santa Clara Valley Medical Center. She then pursued a palliative medicine fellowship at Harvard Medical School. As part of her fellowship, she spent time in Vietnam helping start a home based palliative care program in Ho Chi Minh City and co-taught an intensive palliative care training course at the Geriatrics hospital in the city of Ha Noi. She joined Kaiser Permanente in 2011 as a full-time palliative care physician. During her time at KP, she has been involved on a regional level in both palliative care and advance care planning (known as Life Care Planning) in program development. She has created the Oncology Supportive Care Clinic to promote palliative care involvement early on in a patient’s cancer diagnosis. She is currently the outpatient palliative care and the Life Care Planning physician lead at Santa Clara Medical Center.
Juanita Inderbitzen: Juanita is a Department Manager at Kaiser Permanente Santa Clara. Juanita Manages the following departments: Senior Health, Memory, Life Care Planning, Skilled Nursing Facility and House Calls. For the past 14 years Juanita has had various positions in the organization from Receptionist to Program Assistant to Learning Specialist. In her Learning Specialist role, she was able to share her knowledge about the importance of completing an Advance Health Care Directive and putting health care wishes in writing. She facilitated information sessions for patients and their loves ones, provided education at Kaiser’s Famers Markets as well as participated in community presentations. Juanita has her bachelor’s degree from San Jose State University.
Marlon John: Marlon is a Senior Health Educator at Kaiser Permanente Santa Clara. Marlon who works with physicians, health educators, and medical center leaders to plan, promote, and evaluate health education topics such as diabetes, tobacco, heart health, and lifecare planning. Before Kaiser Permanente, Marlon worked at Stanford University within the Stanford Prevention Research Center (SPRC), where he was instrumental in revamping the employee wellness program. Marlon holds an MPH (Master of Public Health Nutrition) from Liberty University. Marlon is also a certified personal trainer through the National Strength and Conditioning Association.
